Standard AICEM Short Term Consultancy Agreement - School of Population Health


Conditions of Use

  • This document covers the engagement of consultants on a short term basis (i.e. less than 6 months) as part of the Australian Initiative for the Control and Elimination of Malaria (AICEM).
  • This Agreement must be populated by or in consultation with the School of Population Health and must be approved by Ms Heather Depasquale (Principal Finance Officer) or Ms Edith Mendelle (Director of Operations) or their delegate before being signed. 
  • Amendments may be made to the covering letter to insert the relevant commercial and financial information that is specific to the particular consultancy. No amendments may be made to the legal terms in the letter, the General Conditions forming Appendix 2, or to the Schedules of the document.  If such amendments are made, the agreement is no longer considered ‘standard’ and must be reviewed by the Legal Services Office, and signed by the Chief Operating Officer or the Vice Chancellor.
  • You must use the latest version of the agreement downloaded from the Legal Services website each time.
  • You must obtain any relevant University approvals to enter into this contract.
  • You must comply with UQ’s policies and procedures, including any relating to the conduct of a risk assessment, and UQ’s procurement procedures.

Authorised Signatory

In terms of who is authorised to issue contracts using this standard, please confirm the financial delegation of the person signing – see clause 6.3 of PPL 1.10.03 (Contract Delegations) and the relevant bands in PPL 1.10.02 (Financial Delegations).

As stated above, if amendments are made (other than those permitted above), the contract will need to be signed by the Chief Operating Officer or the Vice Chancellor in order to comply with the current delegations. Please note that the majority of the General Conditions and the Schedules have been incorporated as a requirement of the Period Offer Deed agreed between UQ and DFAT.

Steps when using the Agreement

School of Population Health staff should enter into the contract BEFORE the consultant commences work.

The following steps are should be followed to complete the Agreement:

  1. Complete the editable sections noting the Consultant’s name, address and other details as per the prompts. Please be sure to correctly identify the other party involved and if the Consultant is a company, please include the ABN (Australian Business Number). You will need to obtain a full company name (if there is one) and an ABN. To ensure the correct details have been provided to you by the other party, please use the ABN Lookup service.
  2. Obtain internal approval of the Agreement from Ms Heather Depasquale, Ms Edith Mendelle, or their delegate/s.
  3. Send the completed document to the Consultant for their agreement.  If commercial or financial changes are requested, please ensure you obtain internal approval of these. Once the Agreement is finalised, ask the Consultant to sign two copies and return them to you. The Consultant must return a signed copy of the entire agreement – this may be a scanned signed copy, but the return of a signed cover letter or the signature page alone (without the Appendices and Schedules) is not acceptable.
  4. Arrange for an authorised UQ delegate to counter-sign both copies, and return one original or a scanned fully executed copy of the entire agreement to the Consultant for their records. Please retain a copy of the fully executed agreement for your records and send the original to UQ’s Records and Archives Management Services. The LegalServices Office does not keep copies of signed standard documents.

If the School or the Consultant wishes to amend the legal terms in the letter, the General Conditions in Appendix 2, the Schedules, or if you have any legal questions regarding Agreement, please contact the Legal Services Office.
