Standard Student Placement Facilitation Agreement - School of Medicine


Conditions of Use

  • This document covers the engagement of Facilitators who will be placing students with third party placement providers.
  • This Agreement must be populated by or in consultation with the School of Medicine and must be approved by the Senior International Partnerships Officer (currently Ms Georgia Baker) or their delegate before being signed. 
  • Amendments may be made to the covering page to insert the relevant information that is specific to the particular Facilitator. No amendments may be made to the legal terms of the Agreement. If such amendments are made, the agreement is no longer considered ‘standard’ and must be reviewed by the Legal Office, and signed by the Chief Operating Officer or the Vice Chancellor.
  • You must use the latest version of the agreement downloaded from the Legal Office website each time.
  • You must obtain any relevant University approvals to enter into this contract.
  • You must comply with UQ’s policies and procedures, including any relating to the conduct of a risk assessment, and UQ’s procurement procedures.

Authorised Signatory

In terms of who is authorised to issue contracts using this standard, please confirm the financial delegation of the person signing – see clause 6.3 of PPL 1.10.03 (Contract Delegations) and the relevant bands in PPL 1.10.02 (Financial Delegations).

As stated above, if amendments are made (other than those permitted above), the contract will need to be signed by the Chief Operating Officer or the Vice Chancellor in order to comply with the current delegations.

Steps when using the Agreement

School of Medicine staff should enter into the contract BEFORE the Facilitator starts providing services to UQ.

The following steps are should be followed to complete the Agreement:

  1. Complete the editable sections noting the Facilitator’s company name, address and other details as per the prompts. Please also include the company’s ABN number. Please be sure to correctly identify the other party involved. If the Facilitator is a company, you will need to obtain a full company name (if there is one) and an ABN (Australian Business Number) or equivalent if they are an overseas-based organisation. To ensure the correct details have been provided to you by the other party, please use the ABN Lookup service.
  2. Obtain internal approval of the Agreement from the Senior International Partnerships Officer, or their delegate/s.
  3. Send the completed document to the Facilitator for their agreement.  If commercial or financial changes are requested, please ensure you obtain internal approval of these. Once the Agreement is finalised, ask the Facilitator to sign two copies and return them to you. The Facilitator must return a signed copy of the entire agreement – this may be a scanned signed copy, but the return of a signed cover page or the signature page alone (without the terms and conditions) is not acceptable.
  4. Arrange for an authorised UQ delegate to counter-sign both copies, and return one original or a scanned fully executed copy of the entire agreement to the Facilitator for their records. Please retain a copy of the fully executed agreement for your records and send the original to UQ’s Records and Archives Management Services. Legal Services does not keep copies of signed standard documents.
